Has anyone ever tried to connect the Ultimarc Spintrak via USB to a Mac running SDLMAME?
Is it working?

I'd ask Ultimarc support. If it's USB class-compliant (doesn't require drivers on Windows) then it should work on Mac/Linux as well. I don't know how macOS deals with multiple mice though, so that end of things might get "interesting".

I did some test using 2 usb mouses connected to my mac, and also if the "multimouse" oprtion is set to "1" inside the "mame.ini" file, SDLMAME is still recognising only 1 device, but if you play alone it could be not real problem.
In some ways it's also convenient, for example, I can actually control the Analogue Dial using the in internal trackpad of my Mac Book when I'm travelling or the external USB mouse when I'm on my desk.

The main question is if the Ultimarc Spintrak is recognised as a mouse with only 1 axis.
Watching this video (7:13) it seems like that:
