But it does run it, actually I don't even know if there's a VESA option for Doom, and checking its default.cfg there's no line setting that resembles a video mode ...

Doom is purely 320x200x256, you don't need SVGA to run it.

I can see why you used a preloaded CHD hard disk. For years people recomended ct486 to run PC games, I've been always using at486. I tried to boot ct486 with msdos5_35, msdos6, msdos62u and msdos62a from the softlist, however it hangs forever before the blue setup screen appears. That's not a problem with at486 and it seems the issue goes back from at least 0.257 (last time before today I upgraded MAME). I'll see if I can find the regression and fill a bug report in case there isn't any or someone else hasn't done it.

ct486 has been busted for ages when running softlist entries. It was the go-to PC driver in the MESS days, but seemingly not anymore, and not for a while at that.

Bob Zed uses ct486 for all the PC softlist stuff in his videos. Maybe outdated NVRAM or CFG or something?

All this time I've been testing with default settings in a clean folder except for ROM locations. So far I've tested all the way back to 0.181 and the result is constantly the same. Yet, I've found out something that could be useful. Any of the MS-DOS disks listed above boot on ct486 when you press any key every two or so seconds while the disk is booting. If you do not the system hangs into an infinite loop for every MAME version so far. I'll try to test more older MAME versions and see if I find something else.
