Thanks for the pointer to XM6. I've been able to create a user disk with it.

I did the following:
1) create a blank "userdisk" floppy (tools menu, apply logical formatting)
2) Boot Akumajou with disk B in drive 0 and the userdisk in drive 1
3) Choose the yellow option (default), press enter and it will copy for a minute from drive 0 to 1. User disk created.

To use the userdisk, you boot Akumajou Dracula with floppy A in drive 0 and the userdisk in drive 1. It will then skip the japanese screen (that says to create a userdisk) upon starting a game and go directly to entering a name for player 1. More importantly, when you die, it will add a "save" option to the "continue/end" screen. In XM6 pressing save works and brings you to the main screen. Your saved game is then available in the name selection screen upon game start.

Unfortunately for MESS, it's confirmed that write support is broken. After pressing "save" it will throw an error:
